You work in Jones storage yard, Llandudno and have been asked to tidy up the pallets lying around the yard. As you try and place a pallet on top of a stack you misjudge the height and the pallet tips backwards over your head causing you to dislocate your right shoulder as you try to hold on to it. The pallet on top of the stack gets dislodged in the confusion and lands on your right foot causing fractures to the bones of the foot. Use your own name and address and dress appropriately for the occasion including an HV vest.
