Inf2012 002 Safety Alert - Reporting Unsafe Conditions and Acts

On the 16th of January at approximately 2310hrs, A crewmember fell into the dock at Hull Container Terminal and was unfortunately drowned. The incident is still being investigated, however it appears that the crewmember was attempting to use a non-designated and insecure route to move between ship and quay.

Members are advised to review their own arrangements and procedures where appropriate and to re-affirm to employees that:

• all persons on the port should only ever use the safe, properly positioned and rigged gangway to access vessels
• if a gangway is not positioned and rigged correctly it should not be used and should be reported promptly to your supervisor
• everyone has a duty of care to report any unsafe act or near miss including those that involve third party individuals such as ship’s crew members, immediately to your supervisor
