Chemical burn to left eye, closed fracture to the right forearm, shock.

You are one of the cleaning / maintenance team at the North Wales Theatre and Conference Centre and were trying to remove dirty marks from one of the walls using neat bleach when the chair you were standing on tipped. You fall to the floor splashing bleach in your left eye and fracturing your right forearm (radius). Your arm is painful and difficult to move but the pain in your eye is intense and you are unable to open your eye. Use your own your own name and address and dress appropriately for the occasion.
