Open fracture right tibia, bruise to forehead, shock.

You are a member of the Llandudno athletic team and are out on the track practising your hurdling when you misjudge a hurdle and strike your right leg on it. For some reason the hurdle does not give way and you suffer an open fracture to the right tibia and a bruise to the forehead caused when you hit the ground. You are in considerable pain and are unable to stand up. Use your own name and address and dress appropriately for the occasion.
